Colbert Engagement & Support Program

 

Do you reside at Balmoral Home of Southview Manor? Are you interested in moving out of your nursing facility and into the community? The Colbert Engagement & Support Program might be right for you!

Who is Eligible?

Nursing home residents at Balmoral Home and Southview Manor who are interested in moving out of the nursing facility and into their own home in the community through the Colbert program.

How it Works

Those interested will be referred to Trilogy. Trilogy will assist the resident through the process of moving out of the nursing facility and into their own home. The resident will receive a housing voucher to subsidize the cost of rent.

What Support is Available?

Nursing facility residents will have the opportunity to develop independent living skills both before transitioning out of the facility through the Stepping Stones program, as well as through individualized goal setting once they’ve moved into their own home in the community, at no cost to them.
